Our Mission
At Transitions Behavioral Health, we believe that each individual working with us deserves every chance to develop skills and interests to assist them in having a more meaningful day and increased independence in their home and community.
Our Team
Our team of experienced and compassionate mental health professionals includes clinical social workers, case managers, behavior analysts, and qualified mental health specialists from a variety of backgrounds (including music therapy, communication/speech therapy, and education), who are all committed to providing the best quality, individualized care to our clients.
Our Services
We offer a wide range of mental and behavioral health services, including assessment, diagnosis, and treatment of mental and behavioral needs. We specialize in caring for and working with clients with co-occurring mental health and developmental needs and work to integrate quality and compassionate care and services across environments for our clients, including at home, school, and the workplace.

We offer early intervention services at any of our three Early Intensive Behavioral Intervention (EIBI) locations.
Our goal with our early intervention services is to assist our clients in developing skills to gain increased independence at home, school, and in the community. We do this by using facilitated play to decrease behaviors by increasing communication.

We offer therapeutic and positive behavior support services throughout the community.
We work with family members and individuals on:

Our Adult Day Services occur in house at our Duck Creek Location.
We offer a wide variety of tasks to get our clients up and moving daily!
